If you are in need of disability benefits, the following information may be necessary to file a complete Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) claim:

• Details about your treatment (dates, locations and names, addresses and telephone numbers of those who treated you)
• Medications you are currently taking
• Military discharge documents (if you served in the military)
• Tax documents
• Your birth certificate
• Your Social Security card
• Your work history over the past 15 years

You will need to provide additional information if you are applying for Supplemental Security Income (SSI), including information about your income and possessions. This includes paycheck stubs, bank records, insurance policies, automobile registration, burial fund records and information about your housing.
